CTP Program Referral Emails:

ASAM Assessments: asamplacement@crossroadstp.org

  • This email is utilized to receive referrals (internal or external) to have an ASAM Assessment completed. The care coordinators, secretaries, and directors have access to this email.

Circle Referrals: circle-referral@crossroadstp.org

  • This email is utilized to receive referrals to get into the Circle program. The care coordinators and the supervisor have access to this email.

EMERGE (Enter Mindful Evolution and Recovery through Growth and Empowerment) Referrals: emerge_referral@crossroadstp.org

  • This email is utilized to receive referrals (internal and external) regarding patients or referral sources interested in the EMERGE program. The care coordinator, supervisor, and director have access to this email.

ICM (Intensive Case Management) Referrals: icm_referral@crossroadstp.org

  • This email is utilized to receive referrals (internal only) regarding patients’ needs (clothing, hygiene, etc.). While in treatment (residential, outpatient, OTP, Co-occurring, etc.). The intensive case managers and the director have access to this email.
     

IPS (Individual Placement and Support) Referrals: ipsreferral@crossroadstp.org

  • This email is utilized to receive referrals (internal only) regarding patients who need help with obtaining and maintaining employment. The IPS staff to include the supervisor and director, have access to this email.

Men’s IRT (Intensive Residential Treatment) or TRT (Transitional Residential Treatment) Referrals: mrureferral@crossroadstp.org

  • This email is utilized to receive referrals (internal and external) regarding patients who are interested in entering the Men’s IRT or TRT programs. The care coordinator, supervisor, and director have access to this email.

OTP (Opioid Treatment Program, formerly MAT) Referrals: mat.admissions@crossroadstp.org

  • This email is utilized to receive referrals (internal or external) to Opioid Treatment Program (Methadone, Suboxone, and Vivitrol). The care coordinator, counselors, supervisor, and director have access to this email.

 

OTS Referrals: ots_referral@crossroadstp.org

  • This email is utilized to receive referrals (internal or external) for the Offender Treatment Program (MRT, STIRT-CC, etc.). The care coordinator, counselor, supervisor, and director have access to this email.
     

Outpatient/Co-Occurring Referrals: op.admissions@crossroadstp.org

  • This email is utilized to receive referrals (internal or external) for Intensive Outpatient (IOP), Enhanced Outpatient (EOP), Regular Outpatient (OP), and Co-Occurring. The care coordinator, supervisor, and director have access to this email.

Peer Referrals: peerreferrals@crossroadstp.org

  • This email is utilized to receive referrals (internal only) regarding patients who need support from peer specialists. The peers, lead, and director has access to this email.

Recovery Residence Referrals: recoresreferrals@crossroadstp.org

  • This email is utilized to receive referrals (internal or external) for the Recovery Residences (formerly Sober Living). The peers, lead, and director has access to this email.

 

STIRT Men's (STrategic Intensive Remediation Treatment) Referrals: stirrt_m_referral@crossroadstp.org

  • This email is utilized to receive referrals regarding patients who are interested in entering the Men’s STIRT program; or a probation or parole officer who is interested in referring their client to the program. The care coordinator, supervisor, and director have access to this email. This a 21-day offender program, and the referral must come from the probation or parole officer. The LSI (level of supervision inventory) score must be 25 or above.

 

STIRT Women’s (STrategic Intensive Remediation Treatment) Referrals: stirrt_w_referral@crossroadstp.org

  • This email is utilized to receive referrals regarding patients who are interested in entering the Women’s STIRT program; or a probation or parole officer who is interested in referring their client to the program. The care coordinator, supervisor, and director have access to this email. This a 21-day offender program, and the referral must come from the probation or parole officer.
