What is an assessment?

An assessment is a multidimensional tool to identify a person’s appropriate level of care. The assessor will ask a list of questions for each of the six dimensions and identify the risk level based on the answers to the questions. This will determine which level of care is most appropriate.

 

Why do I need an assessment?

The assessment is necessary to assure each person is recommended for the level of care that will best meet their needs.

 

What if I want outpatient treatment?

If you are interested in our outpatient services we can get you started quickly.

 

What if I want residential treatment?

Get an assessment. There are three different treatment levels that focus on different needs. We won’t know what’s best for you until the assessment is completed.

You can contact us at 1-800-304-3758 or via email at info@crossroadstp.org. If you are looking for outpatient services, an appointment will be scheduled. If you are unsure which level of care would be most appropriate, a placement assessment will be scheduled.

 

The secretary will ask for identifying information, including insurance information. After completing your assessment, you will be referred to the treatment level most appropriate for your needs.

 

A case manager or secretary will contact you to schedule an intake appointment.
